About approval of the Procedure and forms of creation and representation of requests for receipt of public information which manager is the Ministry of Culture of Ukraine

According to the Law of Ukraine "About access to public information", the Presidential decree of Ukraine of May 05, 2011 No. 547 "Questions of ensuring with executive bodies of access to public information" and the resolution of the Cabinet of Ministers of Ukraine of May 25, 2011 to No. 583 "Questions of accomplishment of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine, central and local executive bodies" I ORDER:

1. Approve the Procedure for creation and representation of requests for receipt of public information which manager is the Ministry of Culture of Ukraine (further - the Procedure) which is applied.

2. Approve forms of representation of requests for receipts of public information (further - Forms) which are applied.

3. To provide to department of ensuring work of the Minister (G. R. Slivk) placement of the Procedure and Forms on the official website of the Ministry of Culture of Ukraine.

4. To provide to administration (P. M. Kinichenko) submission in accordance with the established procedure of this order on state registration in the Ministry of Justice of Ukraine.

5. To administration (P. M. Kinichenko) to bring this order to permission of deputy ministers, heads of structural divisions of the Ministry of Culture of Ukraine.

6. This order becomes effective from the date of its official publication.

7. To impose control over the implementation of this order on the deputy minister of culture of Ukraine - the chief of staff O. O. Gubina.
