For the purpose of fulfillment of requirements of article 19 of the Law of Ukraine "About access to public information", the Presidential decree of Ukraine of May 05, 2011 No. 547 "Questions of ensuring with executive bodies of access to public information", item 4 of the resolution of the Cabinet of Ministers of Ukraine of May 25, 2011 to No. 583 "Questions of execution of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine, central and local executive bodies" I ORDER:
1. Approve:
forms of requests for receipt of public information which manager is Public service concerning disabled people and veterans of Ukraine who are applied;
2. To provide to department of addresses of citizens and control of accomplishment (T. Shelest) submission of this order on state registration in the Ministry of Justice of Ukraine.
3. Concerning disabled people and veterans of Ukraine (V. Maslakov) to provide to public service placement of forms of requests for receipt of public information which manager is Public service concerning disabled people and veterans of Ukraine, on its official website.
4. To impose control over the implementation of this order on the deputy minister of social policy L. Drozdova.
5. This order becomes effective from the date of its official publication.
