About introduction of amendments to the Order of the Ministry of Finance of Ukraine of November 07, 2011 No. 1395

1. Bring in the form of the tax declaration of property condition and the income approved by the order of the Ministry of Finance of Ukraine of November 07, 2011 No. 1395, registered in the Ministry of Justice of Ukraine on December 27, 2011 for No. 1532/20270 (in edition of the order of the Ministry of Finance of Ukraine of February 03, 2012 No. 84), such changes:

1.1. To state footnote to the name of the tax declaration about property condition and the income in such edition:

"* Appendices 1-7 to the declaration are filled and move only in the presence of the income (expenses) which calculation contains in these amendments".

1.2. In appendix 5 to the tax declaration about property condition and the income to exclude line 09.

1.3. To be reworded as follows appendix 7 to the tax declaration about property condition and the income which is applied.

2. Approve Changes to the Instruction for filling of the tax declaration about property condition and the income approved by the order of the Ministry of Finance of Ukraine of November 07, 2011 No. 1395, of Ukraine registered in the Ministry of Justice on December 27, 2011 for No. 1532/20270, which are applied.
