1. Approve enclosed:

1.1. The application form about provision of one-time financial support.

1.2. Form of the protocol of the commission concerning decision making about provision of one-time financial support to disabled people, detyaminvalida and unemployed needy persons.

1.3. Form of the register of payments (transfer) of one-time financial support to disabled people, handicapped children and unemployed needy persons.

1.4. Report form about use of budgetary funds for payment (transfer) of one-time financial support to disabled people, detyaminvalida and unemployed needy persons.

2. To department of social protection of disabled people (Polyakova O. Yu.) provide submission of this order on state registration in the Ministry of Justice of Ukraine.

3. Repeal the order of Ministry of Labor of Ukraine of June 06, 2008 No. 296 "About approval of document forms for provision of one-time financial support to disabled people and unemployed needy persons" (with changes).

4. This order becomes effective from the date of its official publication.

5. To impose control over the implementation of this order on the deputy minister L. Drozdova.
