In pursuance of provisions of Art. 52 of h (5) the Tax code No. 1163-XIII of April 24, 1997 taking into account changes and amendments I ORDER:
1. Approve the form of the statement for exemption of the income tax of non-profit organizations, according to appendix.
2. Territorial state tax authorities undertake to record statements for exemption of the income tax of non-profit organizations in the special register which will contain the following elements: sequence number; name of non-profit organization; fiscal code of non-profit organization; date of submission of the statement; room and registration date of the statement in territorial state tax authorities.
