1. Approve such which are applied:
1.1. Form of the financial reporting "Balance" (No. form 1) Fund of obligatory national social insurance of Ukraine on unemployment case.
2. To provide to executive directorate of Fund the resolution of board on state registration to the Ministry of Justice of Ukraine.
3. This resolution becomes effective from the date of its official publication.
