1. Approve form of the Tax declaration on tax on the income (profit) of the insurer (further - the Tax declaration) which is applied.

2. Declare invalid the order of State Tax Administration of Ukraine of February 21, 2011 No. 97 "About approval of form of the Tax declaration on tax on the income (profit) of the insurer", registered in the Ministry of Justice of Ukraine on March 21, 2011 for No. 376/19114.

3. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.), to Department of the taxation of legal entities of the State Tax Administration of Ukraine (Pavelko L. G.) to provide in accordance with the established procedure:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. To the State Tax Service of Ukraine (Klimenko O. V.):

in three-months time of day of state registration of this order in the Ministry of Justice of Ukraine to provide development of algorithms of control of acceptance and the automated processing of the Tax declaration;
