About some questions of the organization of activities of interregional territorial authorities of the State inspectorate of Ukraine for safety on sea and river transport
According to the Regulations on the State inspectorate of Ukraine for safety on sea and river transport approved by the Presidential decree of Ukraine of April 08, 2011 No. 447, the order of the Cabinet of Ministers of Ukraine of October 20, 2011 No. 1024-r "About formation of interregional territorial authorities of the State inspectorate for safety on sea and river transport" I ORDER:
1. Approve enclosed:
1.1. Sample of the certificate of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport.
2. To provide to the state inspectorate of Ukraine for safety on sea and river transport:
2.1. Production, issue, storage, accounting and destruction of forms of certificates of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport in accordance with the established procedure.
2.2. Submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.
2.3. Promulgation of this order on the official site of the Ministry of infrastructure of Ukraine.
3. This order becomes effective from the date of its official publication.
4. To impose control over the implementation of this order on the First Deputy Minister Efimenko K. O.

1.1. The certificate of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport (further - the certificate) is the document which certifies position of person. The certificate is issued to safety inspectors of interregional territorial authorities of the State inspection of Ukraine on sea and river transport according to the staff list. The certificate is issued for a period of three years.
1.2. Issue, accounting and destruction of certificates is performed by the State inspectorate of Ukraine for safety on sea and river transport at the expense of means of the government budget, within the allocated budgetary appropriations for maintenance of the State inspectorate of Ukraine for safety on sea and river transport.
1.3. Issue, accounting and destruction of certificates are performed by the official determined by the order of the State inspectorate of Ukraine for safety on sea and river transport.
All damaged or handed-over certificates are liable to obligatory destruction, method which excludes possibility of their reuse about what statements of destruction of certificates are drawn up. Destruction is performed by the commission which structure affirms the order of the State inspectorate of Ukraine for safety on sea and river transport.
1.4. The magazine of certification of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport (appendix 1), register of receipt and use of forms of certificates of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport (appendix 2), register of the lost certificates of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport (appendix 3) are documents for office use and are conducted centralized by the official to who according to the order of the State inspectorate of Ukraine for safety on sea and river transport the obligation of issue, accounting and destruction of certificates is assigned.
Magazines shall be strung together, sheets - are numbered. On inside of cover of the magazine the mark about total quantity of sheets in the magazine which is signed by the head of structural division of the State inspectorate of Ukraine, responsible for it, for safety on sea and river transport and is sealed becomes.
Log entries are performed by ink (ball pen). All details of magazines are filled is legible. The made errors are corrected by deletion of wrong record and introduction new which is confirmed by the signature of person who made correction.
2.1. All entries in the certificate are performed in Ukrainian. The description of the certificate is given in appendix 4 to this Instruction.
2.2. The photograph of the owner of the certificate shall have the sizes of 30 x 40 mm. The photograph is pasted as a witness. The official stamp of the State inspectorate of Ukraine for safety on sea and river transport is set corner of photograph.
2.3. Records are as a witness made only by person responsible for use of certificates, by hand printing letters. Correction in the certificate and its careless registration are not allowed.
2.4. The certificate is signed by the Chairman of the State inspectorate of Ukraine for safety on sea and river transport or person who fulfills its duties.
2.5. The certificate is issued to inspectors under the personal signature in the magazine of certification. In case of delivery of the certificate person responsible for use of certificates, surely explains document purpose, the bases of its use and need of its careful storage about what the corresponding mark in the magazine of certification in the column "Note" becomes.
2.6. In case of dismissal of the inspector the certificate without fail is given in the space of its obtaining for the personal signature of person responsible for use of certificates.
For prolongation of effective period of the certificate the inspector in two weeks prior to the termination of effective period of the certificate hands over it in the space of obtaining for the personal signature of person responsible for use of certificates.
3.1. Inspectors shall make thrifty use of certificates and always have them with themselves during execution of service duties.
3.2. In case of loss or spoil of the certificate its owner shall report about it to the direct head immediately.
The head of interregional territorial authority in writing reports to the Chairman of the State inspectorate of Ukraine for safety on sea and river transport about the fact of loss or spoil of the certificate.
The lost or damaged certificate is considered invalid. Based on the statement of the inspector instead of the lost or damaged certificate new is issued.
On each fact of loss or spoil of the certificate office investigation according to the procedure, established by the current legislation is made. Materials of office investigation are stored at person who is responsible for use of certificates.

The description the certificate of the inspector of interregional territorial authority of the State inspectorate of Ukraine for safety on sea and river transport
The certificate of the inspector of interregional territorial authority (further - the certificate) is book in the curtailed type of 70 x 95 mm in size in firm cover of blue color.
The left and right internal pages of the certificate are made on the protected paper, the background of pages consists of two protective grids (green and blue colors).
On the face of the certificate the images executed by anniversary foil of gold color and text are placed:
in the upper part in the middle - the image of the State Emblem of Ukraine
lower from the image of the State Emblem of Ukraine in the middle - one line of the word "STATE INSPECTION of UKRAINE";
below - two lines of the word "ON SAFETY ON SEA AND RIVER TRANSPORT".
In the upper part of the left inside of the certificate in the center three lines place the text of such content:
"THE STATE INSPECTORATE OF UKRAINE FOR SAFETY ON SEA AND RIVER TRANSPORT"
below - two lines the name of interregional territorial authority.
The words "The STATE INSPECTORATE of UKRAINE FOR SAFETY ON SEA AND RIVER TRANSPORT" and the name of interregional territorial authority are executed by red paint, other text - black paint.
Below on certificate inside at the left seven lines place the text of such content:
"Выдано: фамилия ______________________ имя __________________________ отчество _____________________ Должность ____________________ ______________________________ М.П. Председатель _______________".
Number of the certificate is carried out by printing method.
On certificate inside the photo of the employee of territorial authority of the State inspectorate of Ukraine for safety on sea and river transport of 30 x 40 mm in size contains at the left, the signature of the Chairman is below appended.
The signature and photograph are sealed official the State inspectorate of Ukraine for safety on sea and river transport.
On certificate inside the text of such content is placed on the right:
