1. Approve forms of monthly information on production volumes (realization), level of profitability and selling prices of the made (implementable) goods (works, services):

1) for subjects of the regulated markets rendering services of cellular communication according to appendix 1 to this order;

2) for subjects of the controlled markets (for mobile operators) rendering services in accession of networks of telecommunications of telecom operators, services in the omission of the entering telephone traffic from networks of telecommunications of mobile operators, services in the omission of the entering telephone traffic from networks of telecommunications of operators of the fixed telephone communication according to appendix 2 to this order;

3) for subjects of the regulated markets rendering services of local telephone communication, service in provision of connections of subscribers of local telephone communication with subscribers of cellular communication, service in provision of long distance (intra-zone) telephone communication by means of local telephone communication, service in provision of the international telephone communication by means of local telephone communication according to appendix 3 to this order;

4) for subjects of the controlled markets rendering services in accession of networks of telecommunications of telecom operators at the long-distance and international levels, services in the omission of the entering long-distance, international telephone traffic from networks of telecommunications of the operators of fixed telephony attached at the local, long-distance and international levels, services in the omission of long-distance, international and cellular telephone traffic through networks of telecommunications (the subject of the controlled market) from/on network of telecommunications of the telecom operators attached at the local and (or) long-distance levels (transit) according to appendix 4 to this order;

5) for subjects of the regulated markets rendering services of Internet access for final consumers according to appendix 5 to this order;

6) for subjects of the regulated markets rendering services of primary Internet access (for telecom operators) according to appendix 6 to this order;

7) for subjects of the controlled markets rendering delivery services, to distribution and transfer of printing periodicals according to appendix 7 to this order;

8) for subjects of the controlled markets rendering services in transfer of the registered mail according to appendix 8 to this order;

9) for subjects of the controlled markets rendering services in provision in lease of straight lines (lines) and service in provision in lease of direct couples (lines) according to appendix 9 to this order;

10) for subjects of the regulated markets rendering services of operational maintenance of means of the notification according to appendix 10 to this order;

17) for subjects of the controlled markets rendering services of the mobile Internet according to appendix 17 to this order.

2. (Nurshabekov R. R.) in the established legislative procedure to provide to committee of communication and informatization of the Ministry of Transport and Communications of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan its official publication in mass media and placement on official Internet resource of the Ministry of Transport and Communications of the Republic of Kazakhstan;

3) bringing this order to data of subjects of the controlled markets in the field of telecommunications and mail service.

Declare invalid the order of the Minister of Communications and information of the Republic of Kazakhstan of September 3, 2010 No. 215 "About approval of forms of monthly information on production volumes (realization), level of profitability and selling prices of the made (implementable) goods (works, services) of subjects of the controlled markets in the field of telecommunications and mail service" (registered in the Register of state registration of regulatory legal acts of October 5, 2010 for No. 6520, No. published in the Kazakhstanskaya Pravda newspaper of October 28, 2010 285).
