2. Approve form of "The report on one-time duty" it (is applied).

3. Charge to the Ministry of taxes Nakhchivan ARE, to Department on work with large taxpayers under the Ministry of taxes, to Department of taxes of the city of Baku and territorial administration of taxes: - provide bringing to taxpayers of the forms "Declarations on the Taxes Withheld at Payment Source" and "Report on one-time duty" approved by Items 1 and 2 of this order.

4. Recognize invalid the "Declarations on the Taxes Withheld at Payment Source" form approved by the order on the Ministry of taxes of March 15, 2002 No. A-55.

5. Charge to the Office of the Ministry of taxes to bring (V. Aliyev) copies of the order to all structural divisions of the Ministry of taxes.

6. I reserve control of execution of this order.
