About approval of Changes to the Procedure for forming and submission by insurers of the reporting under means of obligatory national social insurance in connection with the temporary disability and expenses caused by burial

According to Articles 11, "About obligatory national social insurance in connection with the temporary disability and expenses caused by burial" and due to the need of reduction of normative legal acts of Social Insurance Fund on temporary disability in compliance with Procedure for obtaining by insured persons and members of their families of sanatorium treatment which is paid by fund of social insurance for temporary disability, the approved resolution of board of Fund of February 25, 2009 No. 12, registered in the Ministry of Justice of Ukraine on April 14, 2009 for No. 339/16355, board of Social Insurance Fund on temporary disability DECIDES: 27 and 28 Laws of Ukraine

1. Approve Changes to the Procedure for forming and submission by insurers of the reporting under means of obligatory national social insurance in connection with the temporary disability and expenses caused by burial, to No. approved by the resolution of board of Social Insurance Fund on temporary disability of January 18, 2011 4, registered in the Ministry of Justice of Ukraine on March 24, 2011 for No. 392/19130, which are applied.

2. To provide to executive directorate of Social Insurance Fund on temporary disability state registration of this resolution in the procedure established by the legislation.

3. To executive directorates of departments of Fund to bring this resolution to permission of district, interdistrict, city executive directorates of departments of Fund and insurers.

4. This resolution comes into effect from day of its official publication.

1. In the Section VI:

state Item 6.8 in such edition:

"6.8. In the column "From Total Quantity of Permits It Is Issued" (columns 15 - 24) are specified quantity and the amounts of partial payment of cost of permits with effective periods of reporting quarter which are displayed by the accruing result in line 5 "It is received from partial payment for permits" tables I of the report on the F4-Social Insurance Fund form on Chamber of Commerce and Industry";

in Item 6.9 figures "23, 24" to replace with figures "25, 26".

2. To state appendix 2 to the Procedure in such edition:

"Appendix 2

to the Procedure for forming and submission by insurers of the reporting under means of obligatory national social insurance in connection with the temporary disability and expenses caused by burial
