About modification of forms of the tax declarations of the payer of the single tax approved by the Order of the Ministry of Finance of Ukraine of December 21, 2011 No. 1688

According to Item 296.9 of Article 296 of Chapter 1 of the Section X_V of the Tax code of Ukraine, the subitem 64 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the Presidential decree of Ukraine of April 08, 2011 No. 446, I ORDER:

1. Make changes to form of the tax declaration of the payer of the single tax - the physical person - the entrepreneur and form of the tax declaration of the payer of the single tax - the legal entity, the Ministries of Finance of Ukraine approved by the order of December 21, 2011 No. 1688, registered on December 27, 2011 for No. 1533/20271, having been reworded as follows them which are applied the Ministry of Justice of Ukraine.

2. (Chmeruk M. O.) and to Department of the taxation of physical persons of the State Tax Administration of Ukraine (Busarev V. V.) in accordance with the established procedure to provide to department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

3. This order becomes effective from the date of its official publication.

4. To impose control over the implementation of this order on the First Deputy Minister of Finance of Ukraine Myarkovsky A. I. and vice-chairman of the State Tax Administration of Ukraine Ignatov A. P.
