About approval of form of the Certificate on state social insurance
For the purpose of ensuring execution of the sixth paragraph of Item 1 of the Presidential decree of the Azerbaijan Republic "About application of the Law of the Azerbaijan Republic "About the individual (personified) accounting in system of the national social insurance" of December 26, 2001 the No. 616 Cabinet of Ministers of the Azerbaijan Republic decides:
1. Approve "Form of the Certificate on state social insurance" it (is applied).
2. To the Ministries of Economic Development, finance, work and social protection of the population together with the State Social Security Fund of the Azerbaijan Republic to provide the offers connected with the specification of the Certificate on state social insurance and its production to the Cabinet of Ministers of the Azerbaijan Republic.
3. This resolution becomes effective from the date of signing.
