About modification of some regulatory legal acts MZ of Ukraine

1. Bring in the Instruction about carrying out obligatory preliminary and periodic psychiatric examinations approved by the order of the Ministry of Health of Ukraine of January 17, 2002 No. 12, registered in the Ministry of Justice of Ukraine on February 01, 2002 for No. 94/6382, such changes:

exclude Item 14.

With respect thereto to consider Items 15-19 respectively Items 14-18;

in the Instruction for filling of the Medical certificate about passing of obligatory preliminary and periodic psychiatric examinations (form No. 122-2/o) given in appendix 3 to this Instruction:

exclude Item 1.

With respect thereto to consider Items 2-8 respectively Items 1-7;

add this Instruction with new Item 8 of such content:

"8. The medical certificate about passing of obligatory preliminary and periodic psychiatric examinations (form No. 122-2/o) is produced according to the technical description.".

2. Bring in the order of the Ministry of Health of Ukraine of October 20, 1999 No. 252 "About approval of the Procedure for issue of the medical certificate for receipt of permission (license) for object of authorization system", registered in the Ministry of Justice of Ukraine on November 05, 1999 for No. 768/4061, such changes:

exclude Item 3.
