3. Approve recommended sample of the statement from the register of licenses according to the appendix N 6 to this order.

Sokolov) and heads of interregional territorial administrations on supervision of nuclear and radiation safety of Rostekhnadzor to post recommended application forms on the websites of central office and interregional territorial administrations on supervision of nuclear and radiation safety of Rostekhnadzor.
