1. This Procedure determines the mechanism of initiation of the petition for purpose of pensions for special merits in front of Ukraine (farther the petition).
2. The petition is initiated by the officials specified in article 8 of the Law of Ukraine "About pensions for special merits in front of Ukraine".
3. Are applied to the petition:
1) for award of pension to person who has special merits in front of Ukraine:
copies of documents which confirm such merits;
the copy of the page of the passport with indication of surname, name and middle name of person in Ukrainian;
2) for award of pension to members of the family of the died person who had special merits in front of Ukraine:
copies of documents which confirm such merits;
the copy of the page of the passport with indication of surname, name and middle name of the member of the family of person in Ukrainian;
copy of the death certificate of person;
documents which confirm the related relations with person (copies of the marriage certficate, the certificate of birth of children, the judgments about establishment of the related relations);
