1.1. Provision "About Procedure for Carrying Out Licensing of Private Medical Practice in the Ministry of Health of the Kyrgyz Republic" (appendix 1);

1.2. Structure of the commission on licensing of the Ministry of Health of the Kyrgyz Republic (appendix 2);

1.3. Form of the protocol of licensing of NGMU (non-state medical institution) and BMO (business medical owner) (appendix 3);

1.4. Application form (appendix 4);

1.5. Specific requirements to objects of the licensee which are necessary for occupation private medical practice (appendix 5);

2. To the chief УЛППиЛ (Eshkhodzhayeva A. S.) provide licensing of private medical practice according to the legislation of the Kyrgyz Republic and the Provision "About Procedure for Carrying Out Licensing of Private Medical Practice in the Ministry of Health of the Kyrgyz Republic".

3. To impose control of execution of this order on the secretary of state P.U.Suyumbayeva.

Regulations on procedure for carrying out licensing of private medical practice in the Ministry of Health of the Kyrgyz Republic

The regulations on procedure for carrying out licensing of private medical practice in the Ministry of Health of the Kyrgyz Republic, (further in the text the Provision), determine procedure and the procedure of licensing of private medical practice in the Ministry of Health of the Kyrgyz Republic. General provisions

1.1. The Ministry of Health of the Kyrgyz Republic (further the Ministry of Health) performs licensing of private medical practice according to the legislation of the Kyrgyz Republic and according to the procedure reflected in this Provision.

1.2. Medical practice of all subjects of non-state health care sector (physical persons and legal entities, irrespective of legal form and pattern of ownership) according to the legislation of the Kyrgyz Republic is subject to the state licensing.

1.3. Licensing of private medical practice establishes compliance of licensees to the licensed requirements determining the minimum level of resources (personnel, rooms, communications, set of the equipment, the tools) necessary for implementation of medical activities, licensed check of specialists and carrying out certification in the specialty.

1.4. The decision on issue (refusal in issue) licenses is made by the licensed commission of the Ministry of Health based on the conclusion (protocol) of specialists in licensing. Decisions on issue (refusal in issue) licenses it is drawn up by the order of the Ministry of Health based on the conclusion of the licensed commission.

1.5. The procedure for licensing of private medical practice includes the following elements:

1.6. Control of private medical practice from the licensor is exercised throughout all effective period of the license.

1.7. The license is nominal (inaliennable), transfer to its other legal entity or physical person and copying from it is forbidden.

1.8. The license is granted on the licensed type of medical practice specified in the statement of the licensee. In the granted license are specified this the licensee and the address of object.

1.9. The license is granted signed by management of the Ministry of Health according to the order "About Distribution of Obligations of Management of the Ministry of Health of the Kyrgyz Republic".

1.10. Foreign legal entities or physical persons obtain licenses in the same order on condition of nostrification of the diploma about the higher medical education in the procedure established by the legislation.

1.11. In case of reorganization, change of the name of the legal entity, the address of activities, change of type of medical activities or introduction of additional medical and sanitary services the license is renewed.

1.12. License effective period at least 2 years. License renewal is performed according to the legislation of the Kyrgyz Republic.

2. Documents acceptance

2.1. For receipt of the license the licensee (legal entities and physical person) represents to the licensor:

1) the Statement for licensing of the established sample;

Copy of the certificate on state registration:

- in bodies of the Ministry of Justice of the Kyrgyz Republic - for legal entities;

- in bodies of the state statistics - for physical persons;

- in the relevant state - for the foreign physical persons and legal entities which are not registered as legal entities or individual entrepreneurs in the Kyrgyz Republic;

The copy of the INN card, the tax ID number of the taxpayer and payer of insurance premiums confirming assignment to the applicant.

For legal entities copy of the Charter of the legal entity;

For physical persons (individual entrepreneurs) performing private medical practice personally and for the specialists (medical personnel) working on hiring for the legal entity or for the individual entrepreneur:

- The diploma (the verified copy of the diploma) about the termination of the highest (average) medical educational institution;

- the certificate of the termination of clinical internship (internship) for doctors;

- the verified copy of the certificate of passing of enhancement or advanced training by the declared types of medical activities, for the last 5 years;

- the service record - confirming professional qualification and length of service of 5 years on the declared types of activity.

Receipt on payment for licensing;

7) For foreign citizens - the document confirming passing of nostrification of educational documents in the procedure established by the legislation;
