Sample of the identity certificate of the seaman of the Republic of Kazakhstan and requirement to its protection

1. The identity certificate of the seaman of the Republic of Kazakhstan (further - ULM) is made of high-quality materials, considering special working conditions in the sea, according to the requirements of International Civil Aviation Organization containing in part 3 of the document 9303 (the sixth edition, 2006), and suitability for machine reading according to the international requirements and standards shown to machine-readable travel documents.

2. The ULM sizes - 84 x 118 millimeters. Format of the laminated ULM form - 88 x 125 millimeters.

3. Texts in ULM are made in the state and English languages.

4. The ULM form has front (according to the drawing 1) and turnover (according to the drawing 2) the parties. Color tone of the form - blue.

On the face of ULM the State Emblem of the Republic of Kazakhstan, document type and code of the Republic of Kazakhstan, surname, name, middle name (are placed in case of its availability), birth date and birth place, nationality, floor, personal identification number of the seaman, the facsimile and the personal signature of the owner of ULM in the printed type.
