Declare invalid the order of the Ministry of social policy of Ukraine of October 12, 2011 No. 394 "About approval of form of the reporting "About ensuring separate categories of the population with technical and other means of rehabilitation" and Instructions for its filling", registered in the Ministry of Justice of Ukraine on October 31, 2011 for No. 1250/19988.
3. (M. Shambir) to provide to department of provision of pensions and social protection of disabled people:
3.1 submission of this order on state registration in the Ministry of Justice of Ukraine;
3.2 representation of statistics of Ukraine to Public service of summary information in general on Ukraine annually till March 15 for last year.
