1. Approve:
form of the certificate of payment of ecological tax for utilization of vehicles out of service according to appendix 1;
form of the certificate of exemption of ecological tax for utilization of vehicles out of service according to appendix 2.
2. Before implementation of issue of certificates of payment of ecological tax for utilization of vehicles out of service and about exemption of ecological tax for utilization of vehicles out of service as forms of the strict reporting bodies of the income and charges when importing vehicles to customs area of Ukraine and producers during realization of vehicles issue the specified references on the samples approved by this resolution.
3. This resolution becomes effective from the date of publication.
