1. Approve:

form of the form of the certificate of the taxpayer of the Republic of Kazakhstan for legal entities, branches, representations according to appendix 1 to this order;

form of the form of the certificate of the taxpayer of the Republic of Kazakhstan for physical persons according to appendix 2 to this order.

2. Recognize to invalid:

1) the order of the Minister of state revenues of the Republic of Kazakhstan of September 13, 2000 N 1084;

2) appendices 15 and 17 to the order of the Minister of state revenues of the Republic of Kazakhstan of December 8, 2000 N 1355;

3) the order of the Minister of state revenues of the Republic of Kazakhstan of April 23, 2001 N 481.

3. To impose control of execution of this order on the First deputy the Chairman of the Tax Committee of the Ministry of Finance of the Republic of Kazakhstan Usenova N. D.

4. This order becomes effective from the date of signing.
