1. Bring in the Procedure for maintaining the Unified register of special forms of notarial documents approved by the order of the Ministry of Justice of Ukraine of November 04, 2009 No. 2053/5, registered in the Ministry of Justice of Ukraine on November 9, 2009 for No. 1043/17059 (with changes), the following changes:

Subitem 6.1.1 of Item 6.1 of Chapter 6 to exclude 1).

With respect thereto to consider subitem 6.1.2 subitem 6.1.1;

Items 9.3 - 9.5 Chapters 9 to state 2) in the following edition:

"9.3. Managers of offices of notary public, Public notarial Records Offices or private notaries current of five working days after damage or cancellation of forms perform their destruction about what the relevant statement in duplicate is drawn up.

9.4. One copy of the act of destruction of the damaged or cancelled forms is stored in office of notary public, the Public notarial Records Office, at the private notary, the second copy of the act within five working days goes to the Supplier.

9.5. Destruction of defective forms is performed by the Supplier about what the statement of destruction is drawn up.";

3) in Chapter 10:

exclude Item 10.3.

With respect thereto Items 10.4 - 10.10 to consider respectively Items 10.3 - 10.9;

in Item 10.9 of the word "Department of Notariate and Lawyer Associations" shall be replaced with words "Department of notariate and bankruptcy".
