About approval of form of the reporting of GSPI "The Report on Provision of the Government Public Assistance to Disabled People since the Childhood and to Handicapped Children" and the instruction for its filling 

According to article 3 of the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children", the subitem 5 of item 4 of the Regulations on the Ministry of social policy of Ukraine approved by the Presidential decree of Ukraine of April 6, 2011 No. 389 (with changes), I ORDER:

1. Approve form of the reporting of GSPI "The Report on Provision of the Government Public Assistance to Disabled People since the Childhood and to Handicapped Children" and the Instruction for its filling which are applied.

2. Declare invalid the order of the Ministry of Labour and Social Policy of Ukraine of September 1, 2003 No. 241 "About approval of form of the reporting on provision of the government public assistance to disabled people since the childhood and to handicapped children and the instruction for its filling", registered in the Ministry of Justice of Ukraine on September 18, 2003 for No. 823/8144 (with changes).

3. To provide to department of the government public assistance (Muzychenko V. V.):

1) submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure;

2) representation of statistics of Ukraine to Public service of summary information in general on Ukraine and administrative and territorial units annually till March 1 for the accounting period.

4. To provide to the minister of social policy of the Autonomous Republic of Crimea, heads of structural divisions of social protection of the population of the regional, Kiev and Sevastopol city public administrations:

1) collection, processing and generalization of information on the GSPI form;
