About some questions connected with establishment of monthly pension supplement to persons performing powers of the member of the Federation Council, the deputy of the State Duma of Federal Assembly of the Russian Federation

3. Monthly pension supplement is established by the decision of the Ministry of Labour and Social Protection of the Russian Federation (further - the Ministry) based on the petition from the citizen of the Russian Federation issued in form according to appendix No. 1 and provided to the Ministry or to the Office of the Federation Council, State Duma administration respectively.

3(1). In case of refusal the citizen of the Russian Federation from monthly pension supplement he submits the declaration of abandonment of monthly pension supplement issued in form according to appendix No. 1(1). The passport copy of the citizen of the Russian Federation is enclosed to the specified application.

The declaration of abandonment of monthly pension supplement is submitted according to the procedure provided for filing of application about establishment of monthly pension supplement (further - the statement).
