Procedure for provision and deprivation of the status of person deported on national sign which is applied;

certificate sample which is issued to person deported on national sign according to appendix.

Procedure for provision and deprivation of the status of person deported on national sign

1. This Procedure determines the procedure of provision and deprivation of the status of person deported on the national sign, persons determined by part one of article 4 of the Law of Ukraine "About recovery of the rights of persons deported on national sign".

2. Authorized body to which preparation of the documents necessary for receipt of the status of person deported on national sign is assigned are the district, district state administration, executive body of city council of the city of regional value in Kiev, district in the council city (in case of its education) (further - authorized body).

3. For receipt of the status of person deported on national sign person who considers itself person deported on national sign or his legal representative submits at the place of residence or the place of stay of such person to authorized body the statement for provision of the specified status.
