1. Approve:

1) form of the social contract of activation of family, according to appendix 1 to this order;

2) form of the individual plan of the help to family, according to appendix 2 to this order.

2. Declare invalid the order of the Minister of Labour and Social Protection of the population of February 25, 2014 No. 52-O "About approval of form of the social contract of activation of family" (No. 200 registered in the Register of state registration of regulatory legal acts for No. 9195 and published on October 14, 2014 (27821) in the Kazakhstanskaya Pravda newspaper, on October 14, 2014 No. 200 (28423) in the Egemen Kazakstan newspaper).

3. To provide to department of the public assistance of the Ministry of health and social development of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction on official publication in periodic printing editions and information system of law of Ad_let;
