Paragraphs of the seventh and eighth of Item 12 of the Procedure for consideration of the applications on change of name (surname, own name, middle name) the physical person approved by the resolution of the Cabinet of Ministers of Ukraine on July 11, 2007 No. 915 (The Official Bulletin of Ukraine, 2007, No. 52, Art. 2115; 2011, No. 84, Art. 3078; 2012, No. 71, the Art. 2870), after words "law-enforcement body" to add with words ", the GMS territorial subdivision".

The procedure for maintaining the State register of acts of civil status of citizens approved by the resolution of the Cabinet of Ministers of Ukraine of August 22, 2007 No. 1064 (The Official Bulletin of Ukraine, 2007, No. 65, Art. 2516; 2008, No. 21, Art. 600; 2011, No. 84, Art. 3078; 2012, No. 71, Art. 2870, No. 90, Art. 3651; 2015, No. 50, the Art. 1601) to add with Item 9-1 of the following content:
"9-1. The Ministry of Justice according to the procedure, the established Ministry of Internal Affairs and Ministry of Justice, provides to GMS the information from the Register on state registration of marriage or annulment of marriage (during which the surname is changed), change of name.".
