b) The instruction about procedure for filling and submission of the standard form "Information on Collection for Issue of Permissions to Implementation of the International Motor Transportation Carriages, Reusable Permissions of EKMT, Logbooks to Reusable Permissions of EKMT, Route Books / Route Sheets (Interbus) (Forma IATRI 15)", according to appendix No. 2 to this order.
