According to the subitem 2) of Item 3 of article 16 of the Law of the Republic of Kazakhstan "About the state statistics" I ORDER:

1. Approve the form intended for collection of the administrative data "The Report on Appointment and Payment of Cost Recoveries on Training at Home Children with Disability" according to appendix to this order.

2. To provide to department of the public assistance of the Ministry of health and social development of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction on official publication in mass media and information system of law of Ad_let, and also in the Republican center of legal information for entering into reference control bank of regulatory legal acts of the Republic of Kazakhstan;

3) finishing to regional, the cities of Astana and Almaty of managements of coordination of employment and social programs of the above-stated form;

4) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service of the Ministry of health and social development of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1), 2) and 3) of this Item.

3. To impose control of execution of this order on the vice-Minister of health and social development of the Republic of Kazakhstan Zhakupova S. K.

4. This order becomes effective after ten calendar days after day of its first official publication.

Chapter 1. General provisions

1. This explanation determines single requirements for forming of the form intended for collection of the administrative data "The Report on Appointment and Payment of Cost Recoveries on Training at Home Children with Disability" (further – the Form).

2. In this Form the following basic concepts are used:

1) administrative data – individual quantitative (measured by means of numbers) and (or) high-quality (built by certain principle and (or) sign) data on physical person or legal entity and the data of pokhozyaystvenny accounting created by administrative sources, except for primary statistical and alternative data;

2) the Center of development of manpower – the legal entity created according to the decision of the Government of the Republic of Kazakhstan under authorized state body, providing information and analytical maintenance of the labor market, active measures of assistance of employment, methodological support of employment services, development and maintenance of unified information system of the social and labor sphere;

3) child(children) with disability – person aged up to eighteen years having the violation of health with permanent disorder of functions of organism caused by diseases, mutilations (wounds, injuries, contusions), their effects, violations which leads to restriction of life activity and need of its social protection.

3. The main objective of maintaining this form is implementation of quarterly monitoring of appointment and payment of cost recoveries on training of children with disability at home.

4. The form is provided in "Center of Development of Manpower" Joint-stock company by local executive bodies of areas, cities of republican value, the capital quarterly to the 10th following after the accounting period.

in the column 16 of form the average monthly size, the paid cost recoveries is specified to children training with disability at home since the beginning of year which is determined by division of column 15 into column 14 and on the number of months in the specified period.
