1) the application form about state registration of the termination of separate division of the legal entity;

2) the application form about state registration of changes in the data on separate division of the legal entity containing in the Unified State Register of Legal Entities, physical persons entrepreneurs and public forming.

3. To the technical administrator of the Unified State Register of Legal Entities, physical persons entrepreneurs and public forming to take the measures necessary for implementation of this order.
