The Cabinet of Ministers of Ukraine decides:
Make changes to appendix to the description of the form of the death certificate approved by the resolution of the Cabinet of Ministers of Ukraine of November 10, 2010 No. 1025 "About approval of models of assembly records of civil status, descriptions and samples of forms of certificates on state registration of acts of civil status" (The Official Bulletin of Ukraine, 2010, No. 86, Art. 3026; 2015, to No. 78, the Art. 2601), having stated it in edition which is applied.
