1. Approve:

1) Regulations on the register of wholesale selling prices on medicines and products of medical appointment, procedure for introduction in it for changes which is applied;

2) form of the declaration of change of wholesale selling price on medicine which is applied;

3) form of the declaration of change of wholesale selling price on product of medical appointment which is applied.

2. (L. Konoshevich) to provide to management of pharmaceutical activities in accordance with the established procedure provision of this order on state registration in the Ministry of Justice of Ukraine.

3. Declare invalid the order of the Ministry of Health of Ukraine of September 7, 2012 No. 705 "About approval of the Regulations on the register of wholesale selling prices on medicines and products of medical appointment, procedure for introduction in it for changes and forms of statements about declaring of change of wholesale selling price on medicine or product of medical appointment", registered in the Ministry of Justice of Ukraine on September 24, 2012 at No. 1638/21950.

4. To impose control of execution of this order on the deputy minister N. Lisnevskaya.
