Some questions of licensing of economic activity on provision of services and performance of works of fire-proof appointment

Licensed conditions of implementation of economic activity on provision of services and performance of works of fire-proof appointment

General provisions

1. These Licensed conditions determine the exhaustive list of the requirements obligatory for accomplishment by the licensee, and the exhaustive list of the supporting documents enclosed to the application for receipt of the license for implementation of economic activity on provision of services and performance of works of fire-proof appointment (further - the license).

2. In these Licensed conditions terms are used in such value:

the office building - the room (office) and/or the building (construction) which belong to the subject of managing on the property rights or leases in which its governing body is located in case of education and have the postal address;

installation - complex of transactions on installation, connection of separate components, nodes and aggregates, adjustment and conducting check of working capacity;

services of fire-proof appointment - evaluating (examinations) fire-proof condition of the newly created companies and real estate objects (buildings, constructions, rooms or their parts);

works of fire-proof appointment - installation of systems of the fire alarm, the notification about the fire and managements of evacuation of people, the equipment for transfer of alarming notices, fire extinguishing systems (water, foam, gas, powder, aerosol), systems of antismoke protection, maintenance of the operational validity (maintenance) of such systems, observation of fire protection systems, maintenance of emergency firefighting equipment (water, water foam, powder, gas fire extinguishers), fire protection (fireproof impregnation deep or surface, fireproof treatment (painting, plaster, winding, facing), fireproof filling).

Other terms are used in the value given in the Law of Ukraine "About licensing of types of economic activity" and other legal acts.

3. For obtaining, renewal of the license, suspension and renewal of action of the license fully or partially, expansions of implementation of type of economic activity, cancellation of the license fully or partially on provision of services and performance of works of fire-proof appointment the license applicant, the licensee submits to body of licensing the corresponding application in the form given in appendices 1, 1-1-1-5.

And notified by physical persons entrepreneurs who on the religious beliefs refuse adoption of registration number of accounting card of the taxpayer on it the relevant monitoring body, the passport copy with the corresponding mark of such body also moves.

4. The following documents are enclosed to the application for receipt of the license:

1) the data on observance of production requirements concerning availability of the material and technical resources necessary for implementation of the declared type of economic activity on provision of services and performance of works of fire-proof appointment signed by the license applicant in form according to appendix 2;

2) data on availability of the minimum number of workers (heads and contractors (services) in separate positions, signed by the license applicant, in form according to appendix 3;

3) data on availability of places of implementation of economic activity of persons with disability and other handicapped national groups in form according to appendix 11;

4) data on lack of control of activities of the subject of managing in the value given in article 1 of the Law of Ukraine "About protection of the economic competition" and/or trade relations with residents of the states performing the armed aggression against Ukraine in the value given in article 1 of the Law of Ukraine "About defense of Ukraine" in form according to appendix 12;

5) copies of the documents confirming the property right or other corporeal right of the license applicant to the equipment and the rooms necessary for implementation of the corresponding type of economic activity on provision of services and performance of works of fire-proof appointment according to these Licensed conditions and also copies of the documents confirming compliance of means of the measuring equipment to requirements of the legislation on metrology and metrological activities;
