For the purpose of implementation of the Law of the Kyrgyz Republic "About acts of civil status" the Government of the Kyrgyz Republic decides:

1. Approve the enclosed samples of stamp certificates of birth, marriage, annulment of marriage, adoption (adoption), paternity proof, death, change of surname, name, middle name.

2. Bring in the order of the Government of the Kyrgyz Republic of August 3, 1998 No. 511 "About approval of samples of the stamp certificates granted by bodies of civil registration and samples of the certificates granted in case of registration (re-registration) of legal entities, their branches and representations" the following changes:

- to exclude paragraph two of Item 1;

- to recognize the samples approved by the above-stated resolution invalid.

3. To the Ministry of Justice of the Kyrgyz Republic in accordance with the established procedure:

- approve samples of blank forms of civil registrations, and also application forms for state registration of acts of civil status and procedure for their filling;

- make above-mentioned forms of certificates.
