Based on Item 5 of the Regulations on procedure for forming and maintaining the Register of employment agencies approved by the resolution of Council of Ministers of the Republic of Belarus of October 22, 2016 No. 855 "About some measures for implementation of the Law of the Republic of Belarus "About employment of the population of the Republic of Belarus", and subitem 7.1 of Item 7 of the Regulations on the Ministry of Labour and Social Protection of the Republic of Belarus approved by the resolution of Council of Ministers of the Republic of Belarus of October 31, 2001 No. 1589 "Questions of the Ministry of Labour and Social Protection of the Republic of Belarus", the Ministry of Labour and Social Protection of the Republic of Belarus DECIDES:
1. Establish:
the application form about inclusion of data in the Register of employment agencies according to appendix 1;
the application form about modification and (or) amendments in the Register of employment agencies according to appendix 2;
the application form about exception of data of the Register of employment agencies according to appendix 3;
the application form about issue of the duplicate of the certificate on inclusion in the Register of employment agencies according to appendix 4.
2. This resolution becomes effective since January 1, 2017.
