1. Approve:

1) form of the reference for receipt of privileges by disabled people who have no pension entitlements or the public assistance which is applied;

2) the Procedure for issue of the reference for receipt of privileges by disabled people who have no pension entitlements or the public assistance which is applied.

2. To department of social protection of disabled people (Polyakova A. Yu.) provide submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. I reserve control over the implementation of this order.

4. This order becomes effective from the date of its official publication.
