The Cabinet of Ministers of Ukraine decides:

Bring in the resolution of the Cabinet of Ministers of Ukraine of January 29, 2003 No. 117 "About the Unified state automated register of persons having the right to privileges" (The Official Bulletin of Ukraine, 2003, No. 5, Art. 177; 2004, No. 28, Art. 1888; No. 43, Art. 2841; 2006, No. 8, Art. 439; 2009, No. 54, Art. 1864; 2011, No. 28, Art. 1181; 2012, No. 7, Art. 249; 2014, No. 74, Art. 2097; 2016, No. 95, Art. 3107; 2017, No. 2, the Art. 44) changes which are applied.

Changes which are made to the resolution of the Cabinet of Ministers of Ukraine of January 29, 2003 No. 117

1. In the resolution:

Item 3 to state 1) in the following edition:

"3. To the ministry of social policy:

1) to provide entering into the Register of information it is relative:

persons having the right to privileges on social feature according to the Code of civil protection of Ukraine, the Laws of Ukraine "About the status of veterans of war, guarantee of their social protection", "About the basic principles of social protection of veterans of work and other citizens of old age in Ukraine", "About the status of veterans of military service, veterans of law-enforcement bodies, veterans of National police and some other persons and their social protection", "About recovery of the rights of persons deported on national sign", "About recovery of victims of political repressions in Ukraine", "About the status and social protection of the citizens who were injured owing to the Chernobyl catastrophic crash", "About social and legal protection of the military personnel and members of their families", "About education", "About prosecutor's office", "About the Security Service of Ukraine", "About libraries and library science", "About protection of plants", "About bases of social security of disabled people in Ukraine", "About the victims of Nazi prosecutions" "About the Public criminal and executive service of Ukraine", "About Public service of special communication and information security of Ukraine", "About child care", "About social protection of children of war", "About culture", to Bases of the legislation of Ukraine on health care and retirement pensioners;

students, cadets of not military higher educational institutions who get social grants according to the Laws of Ukraine "About providing organization-legal conditions of social protection of the orphan children and children deprived of parent guardianship", "About increase in prestigiousness of miner's work", "About the higher education", "About the status and social protection of the citizens who were injured owing to the Chernobyl catastrophic crash", "About the status of veterans of war, guarantee of their social protection", "About child care";

persons who receive government assistance to injured participants of mass actions of civil protest and members of their families according to the Law of Ukraine "About establishment of government assistance to injured participants of mass actions of civil protest and members of their families";

2) to provide information containing in the Register, to the central executive bodies and local government bodies for ensuring provision of privileges to persons having the right to them with observance of requirements of the Laws of Ukraine "About information" and "About personal data protection"";

2) in item 4 of the word "to managements of work and social protection of the population Kiev and Sevastopol of public administrations, district, district in, to structural divisions concerning work and social protection of the population of executive bodies of city councils" shall be replaced with words "to structural divisions concerning social protection of the population Kiev and Sevastopol of public administrations, executive bodies, district, district in, concerning social protection of the population city, district in the cities (in case of their education) councils".

2. In Regulations on the Unified state automated register of persons having the right to privileges approved by the specified resolution:

1) in Item 1:

in the paragraph the second "head departments of work and social protection of the population of the regional, Kiev and Sevastopol city state administrations, managements of work and social protection of the population Kiev and Sevastopol of the state administrations, structural divisions, district, district in, concerning work and social protection of the population of executive bodies of city councils" shall be replaced with words words "structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city state administrations, structural divisions concerning social protection of the population Kiev and Sevastopol of the state administrations, executive bodies, district, district in, concerning social protection of the population city, district in the cities (in case of their education) councils";

Items 2 and 3 to state 2) in the following edition:

"2. The following information is entered in the Register:

concerning exempts - surname, name, middle name, number, month, year of birth, registration number of accounting card of the taxpayer (except physical persons which owing to the religious beliefs refused adoption of registration number of accounting card of the taxpayer, notified on it the relevant monitoring body and have about it mark in the passport) (further - registration number of accounting card of the taxpayer), the address of the registered residence (according to the decision of the commissions formed by local administrations and local government bodies according to item 4 of the Regulations on procedure for appointment and provision to the population of subsidies for expense recovery for payment of housing and communal services, acquisition of the liquefied gas, the solid and liquid oven household fuel approved by the resolution of the Cabinet of Ministers of Ukraine of October 21, 1995 No. 848 (the joint venture of Ukraine, 1996, No. 2, Art. 76; Official Bulletin of Ukraine, 1997, No. 39, Art. 17; 2000, No. 5, the Art. 178), - the address of the actual accommodation), family composition, the characteristic of housing, details of the passport of the citizen of Ukraine and the document confirming the right to privileges, and also category of the exempt, the list of privileges to which he has the right, information on what privileges he actually uses;

concerning students, cadets of not military higher educational institutions who get social grants - full name and code according to EGRPOU of higher educational institution or its separate structural division surname, name, middle name, number, month, year of birth, registration number of accounting card of the taxpayer, details of the passport of the citizen of Ukraine (certificates of birth of the child), the address of the registered and actual accommodation, the basis for receipt of social grant, the size of monthly grant (together with the indexation amount) and the term to which the social grant is appointed;

about persons who receive government assistance to injured participants of mass actions of civil protest and members of their families, - surname, name, middle name, number, month, year of birth, registration number of accounting card of the taxpayer, the address of the registered and actual accommodation, details of the passport of the citizen of Ukraine, the identity document of the foreigner or the stateless person, person recognized in Ukraine as the refugee or person who needs additional protection.

Information on persons having the right to privileges according to the Law of Ukraine "About the status of veterans of war, guarantee of their social protection" is entered in the Register differentially taking into account belonging of person to certain subcategory.

About persons who receive government assistance in connection with serious bodily assault of bodily harms of average weight or slight injuries, beating, tortures to the participant of mass actions of civil protest, are in addition entered in the Register the sequence number specified in the corresponding list which is created by the Ministry of Health by results of forensic medical examination, date and number of acceptance and registration of the order of the Ministry of Health.

About persons who receive government assistance in connection with the death of the family member who participated in mass actions of civil protest the surname, name, middle name of the dead, details of the death certificate of person, details of documents confirming relationship are in addition entered in the Register (marriage certficates, certificates of birth).

3. Structural divisions concerning social protection of the population Kiev and Sevastopol of the state administrations, executive bodies, district, district in, concerning social protection of the population city, district in the cities (in case of their education) councils (further - authorized bodies):

will organize collection, systematization and storage of information specified in Item 2 this provision and provide its automated use for control of data which move the companies and the organizations providing services for calculations for the services provided to exempts, and carrying out payment of social grants and government assistance to injured participants of mass actions of public protest and members of their families;

record exempts by forming on each exempt of personal accounting card in the 1 privilege form in which registration number of accounting card of the taxpayer is used;

record receivers of social grants and government assistance to injured participants of mass actions of civil protest and members of their families by forming on each person of personal accounting card in the 1 help form in which registration number of accounting card of the taxpayer or details of the passport of the citizen of Ukraine, the identity document of the foreigner or the stateless person, and also person who is recognized in Ukraine as the refugee or person who needs additional protection is used;

enter in the Register the corresponding amendments in case of recognition invalid or suspensions of action of separate regulations of legal acts based on which exempts receive privileges;

provide consultations to exempts, injured participants of mass actions of public protest and members of their families, the companies and the organizations rendering services.";

3) in Item 5:

word in paragraph one of "identification number in the State register of physical persons taxpayers and other obligatory payments" shall be replaced with words "registration number of accounting card of the taxpayer";

word in paragraph four "The state register of physical persons taxpayers and other obligatory payments" shall be replaced with words "to base of personal data - the State register of physical persons taxpayers";

In paragraph three of the subitem 3 of Item 11 of the word "head department of work and social protection of the population of the regional, Kiev and Sevastopol city state administrations" shall be replaced with words 4) to "structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city state administrations".
