According to Article part fifteen 4, to part two of article 5 of the Law of Ukraine "About the basic principles of the state supervision (control) in the field of economic activity", article 41 of the Law of Ukraine "On regulation of town-planning activities" I order:
1. Approve the unified form of the statement which is drawn up by results of conducting scheduled (unplanned) inspection of observance by the subject of managing of licensed conditions of implementation of economic activity on construction of facilities which on class of effects (responsibility) belong to objects with average and large effects which is applied.
2. To department of state programs and development of housing construction (Kozlovskaya N. V.), to Legal department (Dorofeyev M. Yu.) together with the State architectural and construction inspection of Ukraine (Kudryavtsev A. V.) provide submission of this order on state registration in the Ministry of Justice of Ukraine.
3. This order becomes effective from the date of its official publication.
