These Rules of filling of columns (headings) of the book of MDP determine procedure for filling of the book of MDP on customs area of Ukraine on goods which are transported between customs with observance of conditions of accomplishment of MDP.
1. The first (title) page of cover of the book of MDP is filled in thus:
headings (further - graphs) 1-5 - warranty consolidation which issues the book of MDP.
Corrections can be made to these columns by the same warranty consolidation and shall be certified it by special seal;
column 6-12 - the holder of the book of MDP.
Corrections and changes in these columns, except the column 8, are brought only by the official of customs authority, certified it by the signature and print of LNP. Corrections and changes are made to column 8 according to the procedure, determined in Procedure Item 4.6.3.
In the column 1 "It Is Valid for Acceptance of Loads by Customs of the Place of Sending to (Inclusive):" date to which the book of MDP can be shown to customs of sending (effective period of the book of MDP) is specified.
In the column 2 "It Is Issued (Whom)" the name of warranty consolidation which issued the book of MDP is specified.
In the column 3 "Owner" the name and the complete postal address of the holder of the book of MDP are specified.
If the holder of the book of MDP is subject of foreign economic activity - resident, then in the graph its code according to the Unified state register of the companies and organizations of Ukraine (further EGRPOU) created according to the resolution of the Cabinet of Ministers of Ukraine of 22.01.96 of N 118, or identification code according to the State register of physical persons - taxpayers is specified.
On the terms of MDP only the holder of the book of MDP specified in column 3 can transport.
