Being guided by item 4 of the Section XV of the Law of Ukraine of May 22, 2003 N 849-IV "About introduction of amendments to the Law of Ukraine "About the Government budget of Ukraine for 2003" and some other legal acts",

I ORDER:

1. Approve the Declaration form of the maximum retail prices established by the producer or the importer on excise goods (further the Declaration) and the Procedure for creation of the declaration on the maximum retail prices established by the producer or the importer on excise goods which are applied.

2. Approve Amendments to the Procedure by customs authorities of control of payment by taxpayers of the value added tax and the excise duty which are collected when importing (peresylaniye) of goods and other objects to customs area of Ukraine, approved by the general order of State Tax Administration of Ukraine and the State Customs Service of Ukraine of March 16, 2001 N 109/188 and registered in the Ministry of Justice of Ukraine on March 22, 2001 for N 261/5452, which are applied.

3. To department concerning administration of the excise duty and control of production and the address of excise goods (Radchenko A. P.):

3.1. Submit this order on approval to Committee of the Verkhovna Rada of Ukraine concerning finance and banking activity.
