I ORDER:

1. Approve form of the Book of accounting of the income and expenses of small business entity - the legal entity which applies simplified taxation system, accounting and the reporting, and the Procedure for its maintaining that is applied.

2. To the chief of Head department of the direct taxes Fuhrman A. P. provide provision of this order on state registration in the Ministry of Justice of Ukraine.

3. To the head of department in 10-day time after state registration of the order to provide with Kovalenko V. V. cases replication it and posylaniye to State Tax Administrations in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol.

4. To impose control of execution of the order on the vice-chairman Operenko G. M.
