1. Approve enclosed:

2) the Instruction for filling of form of the reporting No. 1 queue "The report on the number of the children staying on the registry for the device in preschool educational institutions".

2. Extend the Form and the Instruction specified in Item 1 of this order in the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations, departments (managements) of formation of district public administrations and executive bodies of city councils.

3. Determine that providing form with the forms and copies of the Instruction specified in Item 1 of this order is assigned to the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations.
