I ORDER:

1. Approve the Procedure for execution of the form of the certificate on state registration of the legal entity and the form of the certificate on state registration of physical person entrepreneur which is applied.

2. (Efremov O. V.) in five-day time to provide to department of registration and licensing provision of this order on state registration in the Ministry of Justice of Ukraine.

3. To impose control of execution of this order on the vice-chairman Tretyakov S. I.

4. This order becomes effective at the same time with entry into force of the Law of Ukraine "About state registration of legal entities and physical persons entrepreneurs".
