About modification of the application form about purpose of all types of the public assistance, compensations and privileges

According to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 No. 423 (with changes), for the purpose of reduction of the regulatory legal act in compliance with the current legislation I ORDER:

1. Bring in the application form about purpose of all types of the public assistance, compensations and privileges approved by the order of the Ministry of social policy of Ukraine of April 21, 2015 No. 441, registered in the Ministry of Justice of Ukraine on April 28, 2015 for No. 475/26920, the following changes:

To replace 1) in the text of the word "the disabled person since the childhood", "handicapped child" and "disabled person" in all cases and numbers respectively with the words "person with disability since the childhood", "child with disability" and "person with disability" in the corresponding cases and numbers;

Item 4 after the paragraph of the seventh to add 2) with the new paragraph the eighth the following content:
