1. Registered in the Ministry of Justice of Ukraine on April 28, 2015 for No. 475/26920, to add Item 1 of the application form on purpose of all types of the public assistance, compensations and privileges approved by the order of the Ministry of social policy of Ukraine of April 21, 2015 No. 441, with new line of such content:

2. To provide to management of family and social support of the population (V. Muzychenko) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.
3. This order becomes effective from the date of its official publication.
4. To impose control of execution of this order on the deputy minister L. Popova.
