Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution on legal entities (except the small business entities listed in part four of Item 1 of Instructions on filling of form of the state statistical reporting 12-is (construction) "The report on accomplishment of the contract works" approved by this resolution), the separate divisions of legal entities performing works on agreements (contracts) of the construction contract.
