About modification of the Procedure for provision and deprivation of the status of person deported on national sign

The Cabinet of Ministers of Ukraine decides:

Bring in the Procedure for provision and deprivation of the status of person deported on national sign, No. 643 approved by the resolution of the Cabinet of Ministers of Ukraine of November 12, 2014 (The Official Bulletin of Ukraine, 2014, No. 96, the Art. 2762), changes which are applied.

Changes which are made to the Procedure for provision and deprivation of the status of person deported on national sign

1. Items 2 - 4 to state in the following edition:

"2. Authorized body to which preparation of the documents necessary for receipt of the status of person deported on national sign is assigned are the district, district state administration, executive body of city council of the city of regional value in Kiev, district in the council city (in case of its education) (further - authorized body).

3. For receipt of the status of person deported on national sign person who considers itself person deported on national sign or his legal representative submits at the place of residence or the place of stay of such person to authorized body the statement for provision of the specified status.

Are enclosed to the application:

copies of pages of the passport of the citizen of Ukraine or the temporary certificate of the citizen of Ukraine which contain surname, name, middle name, place of residence;

passport copies of the citizen of Ukraine and the certificate of registration of the residence (for persons having the passport in the form of card);

two photographs of 3 x 4 centimeters in size;

copies of other documents confirming belonging to number of persons deported on national sign (with presentation of the original). Can be such documents:
