1. Approve:

1.1. Application blank form of issue to the foreign citizen or the person without citizenship of the work permit arriving to the Russian Federation according to the procedure, requiring receipt of a visa (Appendix 1).

1.2. Form the application blank about issue to the foreign citizen or stateless person of the work permit arriving to the Russian Federation according to the procedure, not requiring receipt of a visa (Appendix 2).
