About approval of procedure for issue of the medical certificate about the child with disability aged to 18 years

 In pursuance of the Law of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children" I ORDER:

1. Approve procedure for issue of the medical certificate about the handicapped child aged up to 18 years it (is applied).

2. To the Minister of Health of the Autonomous Republic of Crimea, chiefs of managements of health care regional, Head department Kiev and managements of health care Sevastopol the city public administrations to provide accomplishment of this order.

3. To provide to the chief of the Center of medical statistics of MZ of Ukraine Golubchikov M. V. the Ministry of Health of the Autonomous Republic of Crimea and bodies of health care of local public administrations with samples of the accounting form 080/about "The medical certificate about the handicapped child aged up to 18 years".

4. Read out invalid the order MZ of Ukraine of 05.12.91 No. 175 "About the list of medical indications and procedure for issue of medical certificates to handicapped children from the childhood to 18 years".

5. To impose control of execution of this order on the Deputy State secretary Goydu N. G.
