1. Approve:

1.1. Form of the reporting N4-f (annual) "The report on precious metals and gemstones which contain in museum objects" (administrative data) (further - the N4-f form) which is applied.

1.2. The instruction for filling of form of the reporting of N4-f "The report on precious metals and gemstones which contain in museum objects" (administrative data) (further - the Instruction) which is applied.

2. Extend the N4-f form and the Instruction to all museum institutions which are located in the territory of Ukraine, irrespective of subordination.

3. Enact the N4-f form, starting with the report for 2007.

4. Establish such procedure for submission of the reporting under the N4-f form:

4.1. Museum institutions which are in the field of management of district (city) departments of culture and tourism submit the report on the N4-f form to the relevant district (city) departments of culture and tourism.
